Abstract:
PATIENTS PARTICULARSParticulars is defined as details or information about a person,
especially when officially recorded (McIntosh, 2013) . This is the collection of
personal data such as name, date of birth, age, sex, marital status, family and
other relevant information that will help the nurse in rendering holistic care
to the patient.Ms. Somey sala is the patient
for the study. Ms. S.S was born on 28th November 2012, at sunyani Municipal Hospital. She is 6 years
old and comes from Tamale in the Northern Region of Ghana but currently stays
in sunyani in the Bono Region. She resides at the Sunyani night market with
house number BR-145 along the road closer to the traffic light. Ms S.S is born
to Madam fati somey and Mr. Ali Somey.
She is the second child among the five children born to her parents. She
is fair in complexion with a weight of 30kg and height of 1.0 meter. She is a
Muslim. She speaks both Akan and Wangara which happens to be her mother
tongue. Ms. S.S is a class one pupil of Sunyani Presbyterian School. She has no
PHYSICAL ABNORMALITIES. Her folder number is 013592/19.
